Verizon has been producing a data breach report for the last 6 years.  It gives a good snapshot of the changing nature of breaches to data security and the responses.  The report is found here.

In its 2013 report it noted:

• there were 47,000 security incidents.
• 69% of breaches were spotted by an external party
• 76% of network intrusions exploited weak or stolen credentials
• 75% of attacks were opportunistic and the vast majority are financially motivated
• over 50% of insider attacks were by former employees taking advantage of old accounts or backdoors that were not disabled
• 62% of breaches took months to discover.
